The National Contact Centre for Older People (NCCOP) is dedicated to improving the lives of older adults through innovation and collaboration. By working with partners like Age Care Technologies (ACT), we bring cutting-edge digital tools and personalised support services for older individuals, their families, providers of products and services, and the wider community.
NCCOP is proud to partner with Age Care Technologies (ACT) to offer a comprehensive service that addresses the unique needs of older people. ACT’s digital tools help identify and respond to the top threats to health, independence, and wellbeing in older age, including declines in physical and mental health, wellbeing, and environmental threats. Support is tailored to the priorities of older people, focusing on key threats such as falls, cognitive decline, and depression. Research has shown that ACT’s tools can add an average of one extra year of good quality life, reducing the need and cost of health and care services.
Together, we ensure that older adults can access the care and resources they need to age confidently and independently. Our combined effort includes:
• ACT® Assess: A globally validated tool based on over 30 years of research that helps identify individual concerns.
• ACT® Connect: Personalised signposting to the best sources of information and local services for optimising health, independence, and well-being in old age.
• ACT® Report: A summary report for the older person, with a copy for their health records, detailing concerns, priorities, sources of support, carer support services, information resources, social networking, and significant risks.
• ACT® Insight: Data-driven analysis about the needs, risks, quality of life, and resource
requirements to improve healthy life expectancy in ageing populations.
